In the interrogative, one must use auxiliary have + you + been + past participle. "You're" and "your" do not mean the same thing, even though they're pronounced the same. "You're" is the short form of "you are" and "your" is a possessive determiner.


As for why the other options are incorrect:

a. have you ever told - this should be in the passive so that it has the same meaning as the sentence in D. "your" is not a subject pronoun, so it's incorrect.

c.  "being" is the gerund form of "be", which is incorrect to be used with the present perfect tense.

d. Anyone = he/she, therefore it is incorrect to use "have". Again, "your" is not a subject pronoun, yadda yadda yadda.
