Você terá que utilizar o auxiliar DID, em todas os casos (tanto na forma negativa quanto na forma interrogativa). Note que eu abreviei "did not" e utilizei "didn't" em seu lugar, nas formas negativas. Prossigamos:
a) Negative form: Helen didn't decide to come/Helen decided not to come
Interrogative form: Did Helen decide to come?
b) Negative form: That student didn't cause some problems
Interrogative form: Did that student cause some problems?
c) Negative form: They didn't tell me the secret
Interrogative form: Did they tell me the secret?
d) Negative form: We didn't see them at the club
Interrogative form: Did we see them at the club?
a) Negative form: Helen didn't decide to come/Helen decided not to come
Interrogative form: Did Helen decide to come?
b) Negative form: That student didn't cause some problems
Interrogative form: Did that student cause some problems?
c) Negative form: They didn't tell me the secret
Interrogative form: Did they tell me the secret?
