Afirmativa:  

Com verbo to be: sujeito + were/was + complemento. Ex.: I was happy yesterday. She was worried about the exam. They were very tired.

Com os demais verbos: sujeito + verbo principal no passado + complemento. Ex.: I went home from school. She ran into a brickwall. They lost all their teeth.

Negativa:

Com verbo to be: sujeito + were/was + not + complemento. Ex.: I was not happy yesterday. She was not worried about the exam. They were not very tired.

Com os demais verbos: sujeito + auxiliar did + not + verbo principal na base form + complemento. Ex.: I did not go home from school. She did not run into a brickwall. They did not lose al their teeth.

Lembrando que did not vira didn't na forma contraída, e was not e were not viram wasn't e weren't, respectivamente.

Interrogativa:

Com verbo to be: Was/were + sujeito + complemento. Ex.: Was I happy yesterday? Was she worried about the exam? Were they very tired?

Com os demais verbos: Did + sujeito + verbo principal na base form + complemento. Ex.: Did I go home from school? Did she run into a brickwall? Did they lose all their teeth?
