(Se a gente traduzir, has caused fica igual a "causou", e was caused fica igual a "foi causado" então "o problema foi causado pela sua mãe" é o certo da voz passiva.
Mas tivéssemos escrito a frase na voz ATIVA, "sua mãe causou o problema", então usaríamos o "has caused", ficaria "your mother has caused the problem")
b) was painted
(a mesma coisa. "Painted" = pintou. "Was painted" = foi pintado. A voz passiva é quem recebe a ação, então o certo é "foi pintado", pois "pintou" significa que a pessoa pintou, ou seja, ela praticou a ação (voz ativa))
c) was chocked
d) was eaten.
(Só poderíamos usar o had se usássemos "has been eaten", porque has been eaten = foi comida.
"has eaten" = comeu.
Pra resumir, na voz passiva tem sempre que ter o verbo to be antes do verbo principal.

Had been
O "has" sem o "been" sempre será voz ativa!
