Resposta:

a) Did she visit her parents last weekend?

b) Did he clean his room before school?

c) The teacher didn't find the missing exams.

d) Did Tom want to go to the movies alone?

e) Helena didn't offer Henrique a piece of cake.

Explicação:

Para formar perguntas no simple past, colocamos um did antes do pronome e o verbo fica no infinitivo.

E.g.: Tom bought a car yesterday.

Did Tom buy a car yesterday?

Para formar a negativa, colocá-se um didn't ou um did not depois do pronome e o verbo fica no infinitivo.

E.g.: Tom bought a car yesterday.

Tom didn't buy a car yesterday.

Tom did not buy a car yesterday.

Importante: mesmo que os verbos estejam no infinitivo, não se adiciona a preposição to antes dele!
